John George Psychiatric Pavilion AHD Pharmacist, Per Diem, Days, 8hours in Alameda, California

SUMMARY: Under the direction and supervision of the Director of Pharmacy and / or the Clinical Director of Pharmacy, the staff / clinical Pharmacist is responsible for assuring that the individual drug therapy needs of the patient are met. Pharmacy services provided encompasses all patient populations served by the institution (i.e. neonates, pediatrics, adolescents, adults and geriatrics). Other related duties as required.

DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: The following are the duties performed by employees in this classification, however, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.

  1. Interprets, clarifies and corrects medication orders and applicable treatments directly from the chart or chart copy. Communicates with ordering physician as needed.

  2. Maintains patient records to monitor drug therapy for appropriateness and safety consistent with a high level of patient care.

  3. Gather all pertinent clinical data necessary to evaluate appropriateness of therapies.

  4. Assists Nursing with the routine updating of the patient's medication administration record (MAR) as appropriate.

  5. Provides on-site drug information to Medical, Nursing or other hospital professional staff. Serves as an educator of the hospital staff by presentation of clinical conferences, in-services and drug lectures. Provides medication counseling to patients as required by regulation or as requested by patient / patient's family.

  6. Compounds and / or dispenses medications and intravenous admixtures.

  7. Prepares or checks all parenteral admixtures produced in the department. Pre-packages or supervises the pre-packaging of medication not commercially available in appropriate packaging.

  8. Regularly inspects all medication storage areas in patient care units throughout the facility.

  9. Maintains all records on Controlled Substances.

  10. Supervises ancillary pharmacy staff (technicians, messengers) and coordinates work flow.

  11. Participates in investigational drug and research studies.

  12. Interprets and processes all outpatient prescription orders according to legal requirements.

  13. Labels all dispensed medications completely and accurately. Assigns appropriate expiration to all compounded and / or repackaged items

MINIMUM QUALIFICATIONS: Minimum Education: Bachelor's Degree in Pharmacy. Preferred Education: Advance degree or enrollment in advanced degree program. Preferred Experience: Two years' experience in hospital pharmacy or equivalent (residency training). Required Licenses/Certifications: Valid Pharmacist license issued by the California State Board of Pharmacy.
