Sutter Health Hospice Nurse Practitioner (Alameda) in Alameda, California

Under the supervision of the Hospice Medical Director, the Hospice Nurse Practitioner is responsible for providing medical management for hospice patients within their scope of practice and protocols. This will include hospice patient visits at the patient’s place of residence (home or facility) for the purpose of evaluation and symptom management, admission and hospice related visits.



Graduate from an NLN approved nursing program with current licensure in the State of California as a Nurse Practitioner. MS preferred. Current BCLS CPR certification required. Participation in a specialized training program for Hospice or other coursework in death and dying/hospice care preferred.


Recent hospice experience with a minimum of three years experience in home health or hospice preferred. CHAPNP preferred.


Possess critical thinking skills, in-depth knowledge of clinical disease entities common to hospice and palliative care including the pathophysiology, disease progression, pharmacological and non-pharmacological treatment modalities for palliative care, in-depth knowledge of the hospice philosophy, the Medicare hospice benefit, Conditions of Participation, and Local Coverage Determinations under the Medicare Fiscal Intermediary and CHAP


Valid California driver’s license and access to a reliable automobile with proof of liability insurance. Demonstrated clinical assessment skills. Possess excellent interpersonal and communication skills, both oral and written. Must have the ability to function independently and as a member of an interdisciplinary team; ability to exercise independently sound clinical judgment in planning and providing patient care; ability to recognize the special needs of hospice patients and others including family, friends, caregivers in the home. Ability to communicate with patients, family, friends, and caregivers, in an effective, mature, caring manner. Ability to maintain effective and harmonious working relations with internal and external customers. Must be proficient in utilizing technology such as cellular phones, Smartphone or PDAs, laptop computers and have strong computer skills.
