ESSENTIAL DUTIES AND RESPONSIBILITES Include the following:

• To perform all duties as listed on the Task Assignment Plan which will include activities of daily living and assistance with personal hygiene requirements.
• To hold all client information in confidence as mandated by HIPPA guidelines (Health Insurance Portability and Accountability Act of 1996).
• To provide supervision of activities and assist when necessary for protection from physical harm.
• To perform other duties and tasks as directed by a supervisorincluding lifting a minimumof 51 lbs.
• To report all significant changes in the client's circumstances to immediate supervisor within 24 hours of occurrence, or by the next workday.
• To immediately report client falls to agency.
• To report emergency life endangering situations to client's family, doctor and/or health officials within one hour of awareness.
• To watch for safety hazards and act to remove them or take reasonable steps to remove the risk of danger.

The attendant provides care, warmth and support in accordance with the specified plan of care for the client. The attendant will provide services to maintain strengthen and safeguard the client in their own environment.
