Park Nicollet is looking to hire a Registered Nurse to join our Park Nicollet Target Health & Wellbeing Clinic! Position Summary: The Nurse Line Triage nurse works independently providing professional appropriate telehealth nursing services to patients and/or family members. This is done utilizing a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation and evaluation in accordance with the Nurse Practice Act and its recognized professional standards. Park Nicollet and other approved Triage Guidelines and Protocols are used to support assessment and plan of care. Implements Population based standing orders to treat patients when appropriate, based on Park Nicollet Health Services standing orders and policies. Will function as a liaison between physicians, patients, and staff in the delivery of complex telephone care. The individual must be flexible and have strong interpersonal skills. The nurse must be a patient advocate, maintaining the patient's confidentiality and patient rights. The nurse resolves patient concerns or complaints while maintaining a positive, personalized and friendly attitude, addressing patient needs in a timely and appropriate manner. Functions as a mentor to other team members and works collaboratively with team members to identify and resolve department and/or guideline issues through established procedures. Supports a team environment which allows clear communication, positive support and creative thinking. Triage/ Ambulatory Registered Nurse Role Telephone triage coverage 2-3 days per week Lower call volume than other primary care sites Target sites do not utilize call center, so all calls come directly into clinic Refill management using standing orders In-basket work (e-visits, patient message management) Manage daily nurse schedule (with support from nursing team) Lab visits (nurse to perform phlebotomy and process labs or specimens to be resulted onsite or sent out for processing Allergy Shot Injections Provide patient education (injection teaching, BP follow-up, etc.) Walk-In triage patients With red-flag list symptoms (chest pain, shortness of breath, anaphylactic reaction, etc.) Assist with rooming of provider visits as needed Patient outreach for quality metrics (preventative & chronic conditions) Business Travel Consult Management Standing orders for travel medication refills Immunization follow-up Work Schedule: Clinic Hours are Monday- Friday 7a.m. to 5p.m. No Evenings, No Weekends This is for an ONSITE position Required Qualifications: We are open to filling this role with an RN or LPN. Requirements for each are outlined below: Licensed Practical Nurse (LPN): Graduate from an accredited school of practical nursing. Must maintain a current state license of practical nursing. Current BLS certification for health professionals through the American Heart Association or American Red Cross required and maintained. On-line BLS certification is not acceptable. Registered Nurse (RN): Graduate from course of study in nursing resulting in an associate degree, diploma or baccalaureate degree in nursing. Must have and maintain a valid Minnesota Registered Nurse License. Current BLS certification for health professionals through the American Heart Association or American Red Cross required and maintained. On-line BLS certification is not acceptable. Preferred Qualifications: One or more years of broad based nursing experience is required preferably in a clinic, hospital, urgent care or transitional care setting. Telephone Triage Experience is desirable.
