ILLUMINATION FOUNDATION Transitional Flex Case Manager in Orange County, California

Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.
Job Description
The primary purpose of this role will be to provide support as needed in multiple locations and backfill different job vacancies or as required per company demands. This position is a rover position and will support the following positions: Sustainability, Housing Retention Specialists, Housing Case Managers, and Case Managers. The Transitional Flex Case Manager will be well-versed and responsible for providing housing advocacy and specialized support for clients as needed throughout Orange County locations. Most importantly, the Transitional Flex Case Manager role will adhere to any upcoming training and policy changes for all expected roles. The Transitional Flex Case Manager will adapt quickly to change and will become familiar with the demands and expectations of CSS client care housing needs.
The pay range for this position is $24.00 - $26.00 per hour, depending on experience.
Responsibilities
Evaluating client’s need, developing treatment plans, monitoring progress, providing assistance and referrals, following up with weekly goals and completion of action steps
Complete Client Outcome Matrix and utilize KIPU and HMIS to track and maintain accurate case notes and client and program data
Must attempt to meet assigned clients in-person each month. If client is unable-to-contact then there must be at least 3 modes of communication used in the effort to contact the client in the month
Conduct housing inspections to meet HUD Housing Habitability
As needed, collaborate with the Program Management Team by attending weekly meetings to discuss the effectiveness of programs and client progress, and create new strategies to meet client’s level of need and improve treatment plan.
As needed link clients to resources to assist patients’ psycho-social and daily needs such as health care, food, hygiene items, and referrals to other agencies for permanent supportive housing
As needed perform crisis intervention, as necessary, and counsel client with confronting issues adversely affecting their lives, such as mental illness, addiction and abuse
Establish and maintain confidential case files for participants and prepare required statistical reports for program management and evaluation.
Maintain client records according to HIPAA and adhere to all client confidentiality requirements and standards.
Assist clients and any mutual case workers with searching for housing opportunities, obtaining and filling out housing applications, scheduling apartment tours, and the process of lease signings.
Follow up with continued clients to ensure they are satisfied with IF services or seeking additional assistance from the program.
Complete training and development opportunities as determined appropriate by the assigned site supervisor.
Work with the site supervisors to ensure any additional task and compliance issues are being met accordingly to the variation of positions.

Preferred Experience/Minimum Qualifications
Required:
Bachelor’s degree in Social Services, Healthcare, or related field; or equivalent combination of training and experience.
A certification in Case Management (preferred)
Homeless services, case management, mental health, permanent supportive housing experience
Must maintain a current valid CA drivers license and maintain eligibility for company vehicle insurance
Demonstrate excellent observation, problem solving, verbal and written communication skills
Flexibility to adapt to a variation responsibilities or different task depending on company needs
Experience in the area of non-profit housing and/or housing for people with disabilities and chronic health conditions.
At least 2 years of experience working with at-risk/homeless individuals
Proficient computer, communication, and analytical skills
Must be familiar with VI-SPDAT.
Preferred:
Bilingual in English and Spanish.
Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Teams,)
