POSITION SUMMARY:As a member of the Interdisciplinary team, the Licensed Practical Nurse (LPN) outreaches individuals receiving crisis supports in coordination with other members of the mobile crisis team. The LPN provides basic medical assessment and care within the scope of their training/practice and function as a link to Crisis Response Centers and Emergency Departments as needed. The LPN plays an integral role in all services provided by the mobile crisis team, including rapid response to community crises, assessments, crisis stabilization planning, trauma-informed approach, social determinants of health screening and referrals, community engagement, and linking individuals to other services to meet their immediate and long-term needs. The LPN practices within the scope of Pennsylvania's Nurse Practice Act and in accordance with Elwyn's policies and procedures and those of Elwyn's regulatory agencies. DUTIES AND RESPONSIBILITIES: Complete a focused nursing evaluation at the site of the encounter, when dispatched with the Mobile Crisis Team, which includes but is not limited to attaining the client's medical history, current medical concerns, medications, and recent hospitalization and share all pertinent information with the Registered Nurse (RN); if there is an escalation or imminent danger during the initial encounter, immediately engage emergent resources, e.g. contact 9-1-1 to determine if hospitalization is warranted; document all details of the encounter to the client's chart and to the electronic record as soon as possible after the incident Facilitate transfer to the hospital/Emergency Department by communicating details of the encounter with emergency services, updating the RN on the situation, and providing needed documentation to the emergency services team as follow up regarding the encounter, as needed Collaborate with client's current mental health resources and/or primary care resources, therapists, and other members of the Interdisciplinary Team (IDT) to plan and implement the client's therapeutic program of care; document the treatment plan to the client's chart and electronic record Present pertinent data obtained through direct observation during the client encounter including but not limited to client interview, obtaining and sharing client history, physical evaluation and other reports as received by external resources to the IDT to communicate client progress and ensure continuity of care, and present information in a concise and organized manner Report any observed medication and treatment side effects or abnormal laboratory values to the Registered Nurse (RN) immediately for further observation and follow up care, if needed Administer and document medications given, e.g. Narcon or EpiPen, as indicated; ensure that necessary consents have been completed and documented prior to administration Document all behavioral and physical changes in the client's status and other pertinent information in the nursing progress notes and in the electronic health records; communicate all client information to the incoming crisis mobile staff through verbal and/or written methods Order medical and nursing supplies; restock supplies when needed Maintain medical equipment in proper working order; clean equipment and facilitate equipment repair as necessary; wipe down work surfaces at the end of every shift to ensure infection and disease transmission control Demonstrate flexibility, adaptability, and team work when nursing assignments must be modified to ensure program compliance and quality of care to clients Adhere to Elwyn's policies and procedures related to HIPAA and the disclosure of health protected information by maintaining confidentiality of client records, being respectful of the client by speaking to them in private during interviews and updates, whenever possible Complete all Elwyn mandatory educational requirements by the annual due date and attend nursing meetings and in-services, as required, to ensure program compliance, and learn nursing best practices to provide quality client care Submit new/current nursing license, cardiopulmonary resuscitation (CPR) card, driver's license, physical exam, and contact sheet on or before the actual due dates Maintain valid certifications in cardiopulmonary resuscitation (CPR) and first aid to ensure safe and efficient client care and to be in program compliance Perform cardiopulmonary resuscitation (CPR), first aid, crisis intervention and medication administration using agency-trained protocols Perform other duties as assigned IMMEDIATE SUPERVISOR: Clinical Supervisor DIRECT REPORTS: None CONTACTS:  Elwyn consumers and personnel, families/guardians/social services, physicians, health care providers, and service agencies such as pharmacies, restorative community treatment team, and hospitals EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS: Licensed as a Licensed Practical Nurse in the Commonwealth of Pennsylvania One (1) year of psychiatric, medical surgical or chemical dependency nursing experience Knowledge and understanding of program regulating bodies (e.g. Office of Mental Health & Substance Abuse Services OMHSAS, Department of Behavioral Health) preferred Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities Demonstrated strong attention to detail Demonstrated strong time management and organizational skills Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills Excellent planning, communication, customer interaction, collaboration, and documentation skills required Demonstrated ability to work effectively as part of a team Current CPR certification; must be able to perform CPR, first aid, crisis intervention, medication administration, and crisis intervention techniques using agency-trained protocols Demonstrated basic experience with Microsoft Office applications, including Word, Excel, and Outlook Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills Knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record PHYSICAL DEMANDS/ENVIRONMENTAL PROFILE: See Physical Demands Checklist below which represents the general physical demands and environment conditions to which the employee holding this position must meet, in addition to the duties and responsibilities listed above, to successfully perform the essential functions of this job.
