  Job Summary

• Actively uses the nursing process in goal-focused patient care, demonstrating accountability as an individual and team member.

• Synthesizes information from a variety of sources to develop and implement and individualized collaborative plan of care.

• Documents the actions taken to address the plan of care. Works closely with the care team to coordinate patient activities for complete patient continuum of care.

• Provides timely, continuous, age appropriate, collaborative assessment and preparation of patients and families for the next level of care.

• Provides and documents nursing care to patients of all ages.

• Provides care under supervision of Registered Nurse.

  Education and/or Experience

• Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.

• 3 years of experience required.

  Licensure/Certification/Registration

• Licensed as a LPN in the State of South Dakota and/or meeting the requirements of Multi-state licensure required.

• Basic Life Support (BLS) is required upon hire and must be maintained.

  Work Schedule

• Position requires 72 hours worked in a six-week schedule equating to 624 hours annually, not exceeding 1248 hours in a 12-month period.

• Six weekends required annually.

  Pay

  Flat rate pay of $35.00 per hour
