JOB DESCRIPTION

Title: Intake RN Coordinator

Reports To : Director Patient Care Services

Department : HQ Home Care

FLSA Status: Exempt

Purpose : The Intake Coordinator collects, reviewsand confirmpatient referral information and helps to determine initialeligibilityforadmission to HQ Home Care programper agency, regulations,and accrediting body standards. Builds and maintains a positive relationship with customers and provides general intake support. Promotes and fosters ongoing collaborative communication withinternal/external referrers to establish coordinated home care services. Works under general supervision.

Essential Responsibilities :

  1. Primary responsibility for reviewing patient referral information to determine initial appropriateness for schedulingof admission to the home care program per agency/regulatory standards.

  2. Establishes a prompt positive customer service oriented communication pattern with all referral sources and continuously focuses on meeting the needs of the referral sourceto facilitate patient care transition.

  3. Obtains patient insurance and payment information from various sources, and submits for home care authorization.

  4. Distributes referral information to intaketeamand/or supervisory staff on a timely basis to ensure caseassignment and continuity of care.

  5. Provides home care assessments screening and evaluating to determine appropriate home care services for the patients identified by hospitals and community.

  6. Determines customer needs and provides accurate and responsive intake to ensure customer satisfaction and best practice outcomes.

  7. Continuously educates all internal and external customers on the full range of home services that can be provided by HQHC.

  8. Provides appropriate handoff of information to team members at huddle to support a safe and effective transition of care to home.

  9. Participates as weekend on/call on a rotational basis and in an effective manner.

  10. Responsible for the Key PerformanceMetricsestablished by their manager.

  11. Participates in documentation reviewfor compliance and Performance Improvement activities.

  12. Develops an in-depth knowledge of Medicare, Medicaid, Private insurances, HMOs and other contractual payors as well as confirm insurance coverage for payment.

  13. Maintain and Model REACH Values (Respect,Excellence, Accountability, Compassion, Honor).

  14. Demonstrates regular, reliable and predictable attendance.

  15. Performs other duties as required.

Education and Experience Requirements :

  1. Minimum of three (3) years of RN experience. Requires at leastone year of experience in home

care.

  1. Knowledgeable of Medicare, Medicaid, Insurance, and Charity Care guidelines for home care.

  2. Ability to adapt to change and work in a team based environment required.

  3. Effective oral/written/verbal communication and customer service skills.

  4. Acceptance of philosophy and goals of this Agency.

  5. Ability to exercise initiative and independent judgment.

Minimum Knowledge, Skills and Abilities Requirements:

  • Proficient computers skillsincluding Microsoft Word and Excel

  • Proficient EMR skills

  • NYS Driver’s License

  • Proof of car insurance

License , Registration, or Certification Requirement s:

  • Bachelor’s degree in nursing preferred.

  • License and current registration to practice as a RegisteredNurse in New York State required.

  • CPR Certification preferred.
