ABOUT US

Established over 50 years ago, Project Reality partners with Salt Lake County and Utah State to deliver services that impact the underlying causes of chronic disease and improve community health outcomes. We are a fully integrated health clinic providing comprehensive substance use disorder, primary care, and mental health treatment for vulnerable populations not adequately served in traditional health care settings. Our health home model addresses critical health determinants through a team-based approach utilizing medical providers, therapists, case managers, and peer coaches. We are seeking a part-time Board Certified/Board Eligible Family Medicine Physician.

RESPONSIBILITIES

  • Work closely with Psychiatrist and behavioral team to integrate family medicine treatment into comprehensive care plan; review and implement primary care protocols suited to the population served.
  • Serve as a consultant for team Physician Assistants.
  • Provide direct services for challenging acute and chronic disease conditions; interpret lab and ACG results; educate Physician Assistants to advance disease management knowledge and skill level.
  • Carry out special assignments as appropriate to upgrade clinic quality of care.
  • Assist in coordination with Project Reality partners including speciality medical clinics, OB/Gyn providers, residential treatment programs, and the Salt Lake County Jail.
  • Provide back up to Medical Director as appropriate.

QUALIFICATIONS REQUIRED

  • Board Certified/Board Eligible
  • Active UT License and DEA License
  • Must have a strong commitment to a team dynamic as well as the ability to take initiative.
  • Must have good communication and interpersonal skills as well as an ability to effectively relate to all medical staff and patients.
  • Must be able to pass a DHS/BCI background screening
