You're looking for an opportunity to make a difference in your local community by managing patient workflow, including assessment, triage, treatments, and documentation in compliance with nursing standards of care. This is a day shift Monday-Friday, office based position. No weekends or on call hours!! Write your next chapter: Under guidance of the Nursing supervisor, you will assist medical staff with examinations, treatments, and implement medical staff orders. Schedule medical staff visits and appointments, and facilitate new referrals. Review new patient health assessment forms to identify any allergies, physical disorders, or conditions that are relevant to the management of services provided. Prepare and update medical files of patients, document health problems, obtain medication consent, and review medication for any potential side effects associated with medication with patients and family member(s). Assist clients in locating appropriate health services and resources (i.e. lab work, making initial contact with service provider, making appointment for services). Prepare monthly nursing report for medical section of clinical files and report outcomes. Document billable units using the agency scheduler and clinical services software programs. Maintain medical supplies (i.e. aspirin, bandages) and submit supply recommendations and purchase orders. Perform injections, venipuncture's, drug screens, medication education, logging and oversight. Obtain and facilitate the authorization for release of information for Med/Somatic consumers. Review treatment plans and 90-Day review of Med/Somatic goals with patients. Working in a team based environment is important to you. Serve as a liaison between medical staff and patients. Report recommendations for medical examination or further physical health assessment to the clinical staff. You have.

  • HSD/GED
  • Currenly licensed as an LPN (Licensed Practical Nurse) by the state of Ohio
  • A valid driver's license, with the ability to maintain auto insurance
  • CPR/First Aid Certification or willingness to obtain one
  • Ability to regularly use various computer apps including Microsoft Office, our HRIS, EHR, and other online systems, with multiple logins often simultaneously
  • Willingness to submit background check and fingerprinting

Preferably.

  • Associate's degree or higher in nursing
  • One-plus (1+) years of previous experience in a psychiatric or community mental health, working with individuals that have a primary mental health diagnosis and/or co-occurring disorders, highly preferred.
