JOB REQUIREMENTS: Diabetic Caregiver Are you familiar with diabetes and interested in being part of a small growing team? Do you enjoy assisting someone with general cleaning and organization? Do you have a flexible schedule and enjoy working short shifts? Community Horizons is assisting in recruiting the right caregiver to assist a Wisconsin Rapids female to remain healthy in her own home. Wage: \$15.00 per hour. Visits are not to exceed one hour. Hours of Need Tuesday, Thursdays, Saturdays & Sundays at 5:00pm Monday Sunday at 7:30 pm Potential Pick Up Shifts Available Tasks Include: Diabetic Checks (Monitor insulin levels & making sure insulin is being taken as prescribed) Clean/monitor to be sure the CPAP machine is clean and in working order Assist with general cleaning/organization as instructed/as needed by this member. Showering with standby assistance and some hands-on o (only on Mon., Wed. & Fridays at 5:00 pm) Monitor skin integrity and report concerns Ideal Candidate Characteristics: Familiar with diabetes, insulin, and blood sugars Skilled in personal care such as showering, transfers, etc. Consistent and dependable Happy and friendly Someone who listens, goes the extra mile & is family-orientated Able to follow instructions and perform tasks to assist the member the way they desire to be assisted 1 year experience in mental health Has a reliable form of transportation Favorable Background check Regular Schedule within the Home Monday Sunday Daily Diabetic Checks at 8:30,12:00, 5:00 & 7:30 Monday, Wednesday, and Friday - 5:00 pm for showering assistance and diabetic check Location: 1:1 in member\'s own home. Self-direction gives program participants a choice over who provides services and how they receive them. Administer bedside or personal care, such as ambulation or personal hygiene assistance. Perform healthcare-related tasks, such as monitoring vital signs and medication, under the direction of registered nurses or physiotherapists.
